The FCC ID on the card's label is M4Y-0325H. If there's another synonymous FCC ID, that's great - but it's somewhat bad practice to use an FCC ID not labeled on the device (though, yes, I've done the same at times). M4Y-0325NL was also granted about 4 years after.
It probably uses ISL3873 (ISL3873A/ISL3873B), but without confirmation, I'd prefer to leave it blank. If there's a source, cite it.
